Oxytocin and Pitocin in labor
The short answer
Oxytocin is a medicine that makes the uterus contract. The brand name Pitocin is often used for it. It is used to start labor or to make contractions stronger and more regular. 1
The risk is not the medicine itself. It is contractions that come too close together. A baby gets oxygen between contractions, not during them, so contractions that come too often leave less recovery time. More than five contractions in 10 minutes, averaged over 30 minutes, is called tachysystole. 1

What is it?
The uterus is a muscle. Oxytocin makes it contract. Your own body makes it during labor, and the medicine is a synthetic version of the same thing.
It is given through a vein, in a controlled infusion, so the dose can be raised or lowered minute by minute. That is deliberate: the aim is a contraction pattern that moves labor along without overdoing it.
There are two separate uses.
Induction means starting labor that has not begun.
Augmentation means strengthening labor that has started but is not progressing.
There is a third use that is not about labor at all. After birth, oxytocin is the first-line medicine to make the uterus clamp down and stop bleeding. 4 If you were given oxytocin after delivery, that was for bleeding, not for labor.
Why contraction frequency matters to the baby
Blood flows into the placenta between contractions. During a contraction, the muscle squeezes the vessels and that flow drops. The baby draws on the oxygen delivered in the gaps.
Shorten the gaps and you shorten the recovery time. A healthy baby with a well-functioning placenta has reserve and copes. A baby with less reserve does not. That is the whole mechanism. 1
What does standard care look like?
Contraction frequency is monitored. tachysystole is defined as more than five contractions in 10 minutes, averaged over 30 minutes. It is recorded and described as being with or without changes in the fetal heart rate. 1
The dose is titrated. Oxytocin is increased in steps, at intervals, and reduced or stopped when contractions come too often or the fetal heart rate becomes concerning. 2
Stopping or reducing oxytocin is a first-line response. When the tracing becomes concerning, guidance lists reducing or discontinuing oxytocin among the standard measures. 2
Continuous fetal monitoring is used during oxytocin infusion. 1

Labor progress is defined. Guidance sets out what counts as labor arrest and cautions against diagnosing it too early, which matters because oxytocin is often the response to slow progress. 5
What can go wrong?
Contractions too frequent and the dose not reduced. The infusion record shows the rate at every point. The strip shows the contractions. Putting them side by side is the core of this review. 1
tachysystole with a concerning fetal heart rate and no change. Tachysystole together with heart rate changes is the combination that calls for action. 2
Oxytocin continued through a Category III tracing. Reducing or stopping is one of the standard measures. 2
The dose increased past the point of adequate contractions. More contractions is not the goal. Adequate progress is.
Used in a trial of labor after cesarean without regard to the specific guidance. 3
Labor arrest diagnosed too early, leading to augmentation that was not needed. 5
No record of the infusion rate over time, so the pattern cannot be reconstructed.
None of these establishes causation on its own. Professional guidance is explicit that no single finding shows that events during labor caused a newborn brain injury. 6
Which injuries can follow?
Where contractions reduce oxygen delivery enough and for long enough, the associated injuries are:
- Hypoxic-ischemic encephalopathy. 6
- Neonatal seizures. 7
- Cerebral palsy, where injury is significant. 8
- Meconium aspiration syndrome, where a stressed baby passes and inhales meconium.
For the mother, high-dose or prolonged oxytocin is associated with:
- Uterine rupture, particularly with a uterine scar. 3
- Postpartum hemorrhage from a uterus that has been contracting for a long time and then does not clamp down. 4
What does this look like in the records?
The document to ask for by name is the oxytocin infusion record, sometimes called the medication administration record or the labor flow sheet. It shows the starting rate, every change, and the time of each.

What to count on the strip
- Contractions in each 10-minute window.
- Whether the count went above five, and for how long.
- Whether the fetal heart rate changed during those windows.
- Whether the oxytocin rate changed after that.
What to look for in the notes
- Any entry recording tachysystole.
- What was done in response, and at what time.
- When a physician was notified.
- Whether oxytocin was reduced or stopped, and when it was restarted.

Is Pitocin dangerous?
Oxytocin is a widely used and useful medicine. It also has one of the clearest dose-related effects in obstetrics: too much produces contractions that come too close together, which reduces the recovery time a baby has between them. 1 The question in any individual case is what the contraction pattern was and what was done about it.
What is tachysystole?
More than five contractions in 10 minutes, averaged over 30 minutes. It is a defined term and it should be recorded when it occurs, along with whether the fetal heart rate changed with it. 1
I was on Pitocin and my baby is fine. Should I worry?
No. Oxytocin is used in a very large share of labors and the great majority of babies are born well. This page exists for families trying to understand a record after something went wrong, not to suggest that oxytocin causes harm in ordinary use.
They gave me Pitocin after the birth. Was that for labor?
No. Oxytocin after delivery is used to make the uterus clamp down and reduce bleeding, and it is the first-line medicine for preventing and treating postpartum hemorrhage. 4 It is a different use from labor augmentation.
